五、投标要求：

1、投标单位必须是一般纳税人，提供营业执照等工商资料。

2、投标单位必须具有环境检测、评审机构资质认定证书。

3、付款方式排污许可证取证后开具增值税专用发票后付款。

六、标书制作要求

1、第一部分为投标承诺书和法人授权委托书。

2、第二部分为项目报价单。

3、第三部分为交货期及保证措施，主要材料产地、品牌。

4、第四部分为资质单位情况介绍和有效资质。

八、其他

1、投标书数量：正本一份，副本两份。

2、投标保证金：投标单位在投标前以现金或其他方式缴纳不低于2000元的投标保证金，中标方签订合同后自动转为履约保证金，未中标的7天内退还。

3、投标单位应保证所有投标材料真实有效，招标单位如发现投标方的投标材料或合同履行过程中弄虚作假，招标方有权解除合同并不予退还投标保证金和履约保证金。
